MARTHA CARRICK SCHOLARSHIP APPLICATION

The Southwestern Ohio School
Nurses Association Scholarship is available to a SWOSNA member in good standing
who is pursuing a post graduate degree in a program related to school nursing.
The applicant must be employed full time or part time as a
school nurse for at least three years.
Please print the application and complete the questions. The
completed application should be submitted to the President of SWOSNA and will be
reviewed by a committee appointed by the President. E-Mail any questions to

NCSN SCHOLARSHIP
APPICATION
SWOSNA would like to show their support and
encourage school nurses to take the NCSN examination by offering a partial
scholarship to four members each year.
Requirements for Application:

SWOSNA member during the contest year (see
above dates).

Took and passed the NCSN exam during the
contest year (see above dates).

Must have attended one (1) SWOSNA meeting
during the contest year (see
above dates).
Guidelines for the Scholarship Contest:

Only those applicants meeting requirements
set forth on the application will
be considered for the scholarship.

Applications must be mailed or faxed to
the PR chair person.

A maximum of four (4) scholarships will be
awarded during the contest year.

Four complete applications meeting the
above requirements will be drawn at
the September general meeting.

One half (1/2) of the NCSN exam cost for
NASN members will be reimbursed
directly to the applicant (currently $112.50).
